Dr. Goodman is opt out Medicare which means he is not a Medicare provider and you will not be allowed to submit our bills for Medicare reimbursement. If you have supplementary Medicare insurance, ask your insurance company as to how you can have them cover these services. We will give you a form which the insurance company will need to document this status. You can also download the form from the Home Page bar on the right that reads Patient Forms. During your initial visit both you and Dr. Goodman will sign and affirm that you understand that we are not Medicare participants and that you will not be permitted by law to submit our receipts for payment for Medicare reimbursement. You will also be given a signed copy of this form for your own files if you request it.
