An initial appointment is one hour. If you have made an appointment, the office manager will either email you the necessary forms or you can download them from our site at Patient Downloads on the above menu according to your preference.
Please fill out the forms you download as applicable. Bring them with you to the first visit. If you want to share additional information, bring that as well.
Dr. Goodman will read what you bring and make notes. Then he will follow this up with his own questioning to obtain the basic information he needs to help you as well as with a physical examination.
Wear loose clothing to facilitate the physical examination. You will be asked to remove your shoes and socks.
If you need to cancel or reschedule your appointment, please contact us at least one business day in advance. We keep a waiting list for patients and this allows us to offer someone else your appointment time. It is our office policy to charge for a missed appointment and late cancellations. If you fail to give notice for a missed appointment more than twice, we will ask you to find another physician. If you do not cancel in time for the first visit or do not show up for your initial appointment you will not be rescheduled.
